Cogan Ophthalmic History Society 32nd Annual Meeting

Please complete one form for each person attending the meeting. Registration materials and fees must be received by February 20, 2019.
__ Scientific presentations (includes breakfasts and Saturday lunch) and evening social events: $485
__ Only scientific presentations (includes breakfasts & Saturday lunch) NO evening social events: $375
__ Only Friday evening reception and/or Saturday evening dinner (breakfasts and lunch NOT included, though you can pay for them at the time): $135
__ Resident/fellow/trainee/student (full-time) attending scientific presentations +/- evening social event: $75. Those trainees who have an abstract accepted for presentation will have their registration refunded, and will also receive a $25 amazon.com gift card.
